Tylenol, Ibuprofen, Naproxen, Aspirin – Pediatric doses

Acetaminophen 10-15mg/kg/dose po q4-6h; Max daily dose = 4000 mg (i.e. 4 grams)

NSAIDS:
Ibuprofen 5-10mg/kg/dose po q4-6h
Naproxen 5-10mg/kg/dose po q12h (i.e. 10-20mg/kg/DAY po divided q12h): Max: 1000 mg/day. Naproxen = Aleve = BID !
Aspirin
Pain/fever: 10-15 mg/kg/dose po/pr q4-6h; Max dose: 4g/24hr

*In general, children over 12 years and over 40kg may receive adult doses.
Mucolytics

Tylenol, Ibuprofen, Naproxen, Aspirin – Adult doses

Acetaminophen 325-1000mg po q4-6h Max: 4g /day
Ibuprofen 300-800mg q4-q8h. Max: 2400mg /day . 400 mg q4h or 800mg q8h. 400 or less mg can take q4h. Above 400mg is q8h.
Naproxen 250-500 mg po q12h; Max:1500 mg/day x 6 months
Aspirin
– Pain/Fever: 325-650 mg po q4hr prn; Max: 4g/day
– Acute coronary syndrome: 162-325 mg po x 1
– MI / TIA / thromboembolic stroke prevention : 81-325 mg po qd
– Larger doses for arthritis or rheumatic fever.

**Naproxen is safe for adult patients with a history of previous MI.

Seizures in hospitalized patient

Levetiracetam (Keppra)
For both partial seizures, primary generalized tonic clonic seizures, and juvenile myoclonic epilepsy
Levetiracetam 500-1500 mg po/iv q12h;
Start: 500 mg po/iv q12h, increase by 1000 mg/day q2wk; Max: 3000 mg/day

Probiotics

saccharomyces boulardii lyo (Florastor)  250 mg = 1 cap, Oral, BID

*Lasix

80 mg of po lasix = 40 mg IV lasix. The ratio is 1:2.

Antifungal

Micafungin 150mg, IV piggyback, q24hr : Often used to empirically cover fungi in a patient e.g. who has PNA that has been covered with broad antibiotics for days without improvement.
