Contraindications include:

  • Periarticular infection/skin infection overlying injection site
  • Broken skin at the injection site,
  • Septic arthritis,
  • Periarticular or intra-articular fracture,
  • Joint instability,
  • Juxta-articular osteoporosis
  • Severe joint destruction
  • Known hypersensitivity to the agent to be injected.
  • Prosthetic joint (a relative contraindication)
  • Unstable coagulopathy

Before total joint replacements. Don’t do steroid injections in patients waiting for a total joint replacement surgery because this may increase the risk of intraoperative or postoperative infection of that joint. People who receive intraarticular glucocorticoid injection within the three months prior to surgery are at higher risk of infection.

Anticoagulants. Being on anticoagulants isn’t generally considered as a contraindication for joint injection or aspiration as long as it is done with a smaller-gauge needle.
