The National Kidney Foundation and the KDIGO CKD guidelines define CKD as:

GFR <60 mL/min per 1.73 m2 for at least 3 months
OR
-Other evidence of kidney damage such as 1) albuminuria, 2) abnormal kidney imaging, or 3) abnormal kidney biopsy for 3 months or more with implications for health.

Severe Chronic Kidney Disease

Thresholds used to define severe chronic kidney disease include:

  • an eGFR <30 mL/min/1.73 m2,
  • a urine albumin to creatinine ratio >300  µg/mg,
  • persistent acidosis or potassium imbalance,
  • non–iron deficiency anemia with a hemoglobin level <10 g/dL, and
  • evidence of secondary hyperparathyroidism.

End-stage renal disease (kidney failure)

  • GFR below 15 mL per minute per 1.73 m2, OR
  • Need for kidney replacement therapy (dialysis or transplant)
