Sample Case
A previously healthy male neonate (<4 weeks old) admitted with a cc of “Gagging and breathing funny”. 3 days prior to admission, the patient started “breathing funny, making noises like wanting to clear his throat, sneezing, and gagging’. Decreased PO intake and decreased BMs and wet diapers. Two sick contacts.

Labs: CBC; BMP; Influenza A/B Antigen (from nasopharyngeal swab); RSV antigen (from nasopharyngeal swab)
CXR: Perihilar peribronchial thickening. No focal consolidation, effusion or pneumothorax.

Influenza A : positive

Treatment
Admit to peds inpatient for observation
IV Access
IV Fluids: D5 1/2 NS +20mEq/L at maintenance
Bottle or breast ad lib
Oseltamivir 12mg/dose PO BID x 5 days
Vitamin D
