Basal Insulin Dose.
Start with 10 units or 0.2 U / Kg QHS
The 0.2U/kg may be better because it helps you start the right basal dose without being afraid. You can also start at 10 units and then increase.

Patient on Lantus and Premeal Insulin

Lantus QHS + Premeal Insulin (e.g. Lispro 5 units with meals) + Sliding Scale Insulin. That’s a method I have used. Before meals, the patient checks BS. If say 200, that will correspond to a certain amount of glucose on the sliding scale, to that we add the premeal insulin to get the total that is injected before the meal. This helps make it in a way that we don’t give too much premeal insulin without regard to the blood sugar at the time.
