Antibiotics ideal for intravenous-to-oral (IV-to-PO) switch programs include:

  • clindamycin,
  • metronidazole,
  • trimethoprim-sulfamethoxazole,
  • fluconazole, itraconazole, voriconazole,
  • doxycycline, minocycline,
  • Fluoroquinolones: levofloxacin, moxifloxacin, Ofloxacin,
  • linezolid

Also, the following can be used to switch from IV to Po.

Cephalosporins

  • Cefuroxime, cefuroxime axetil, and cefetamet pivoxil 
  • Ceftriaxone and ceftibuten
  • Ceftriaxone and Cefdinir

Macrolides

Azithromycin and clarithromycin can be used when switching from IV to PO antibiotics.

Other antibiotics

Ertapenem can also be taken po.
