• Neuropathic pain: Amitriptyline / Gabapentin / pregabalin; also SNRIs e.g. duloxetine.
    Restless leg syndrome: Requip/ Ropinirole
    -Duloxetine is used by Dr. Toby for a patient who doesn’t do well with Gabapentin. Can use both.
  • Anti-inflammatory: Mobic (systemic) and Voltaren Gel(topical). Ketorolac = Toradol; Meloxicam=Mobic
  • Muscle relaxer: Baclofen, Flexeril (Cyclobenzaprine) 
  • Lidocaine patch
    Opioids (cause itchiness from histamine release. )
  • Norco or Percocet 10-325
  • Morphine 30mg ER po TID (for very bad pain)
  • Fentanyl patch 25 (Dr. Toby recommended to a patient who failed Norco and Morphine TX–these meds didn’t work)
  • Methadone (Let pain management prescribe it)

  • Steroid injections by PCP or at Pain Clinic.
    Others
  • Back brace
  • Physical therapy
  • TENS Unit — insurance only pays it for knee pain or shoulder pain.
