#Tympanic membrane perforation, in children
-Most common cause of perforation is 2/2 to otitis media leading to increased pressure buildup in the middle ear and perforation. May also occur 2/2 to trauma – Direct trauma to the ear, injury from cleaning or another type of instrumentation, or trauma from an explosion.
-Most perforations heal on their own without any need for intervention.
-When otitis externa is present, may use a topical antibiotic suspension (not a solution form) because the suspension has increased viscosity and is less likely to enter the inner ear.
-In perforated TM 2/2 to otitis media, systemic oral therapy recommended over topical antibiotics.
-Pt to keep TM dry. No swimming. Wear a shower cap when showering to prevent water from entering the canal.
