Denies: Abd pain, fever/chills, dysuria, frequency, urgency, hesitancy, suprapubic pain, overactive bladder, urinary incontinence.
Denies: use of feminine hygiene products, soaps, detergents, contraceptive materials, pessaries, sex toys, new medications. No new clothing and no tight-fitting/nonbreathable clothing.
Pt denies Recurrent UTIs and urinary urgency (which may be caused by diminished estrogen).
PMH: Denies hx of DM, immunosuppression, recent antibiotic use. No hx of atopy. No history of DVT or heart problems. No personal history of breast, uterine, or ovarian cancer. No personal history of breast mass, biopsy, atypia or any abnormal breast pathology.
Family history: No family history of breast, uterine, or ovarian cancer. No family history of DVT.
Social hx: Patient denies smoking. Denies diet high in refined sugars.
Sexual hx: No hx of STDs or vaginitis. No new/multiple sexual partners. She doesn’t have sex with women. No unprotected sex. No vaginal douching, barrier contraception, no IUD/diaphragm/spermicide. No orogenital sex.
