VITALS:
All ages: Height, weight, Temp, Resp, Pulse, Head circumference ( < 2- 3 years)
3 years and up: Blood pressure.
12 years and up: Depression screening (Every 3 months)
13 years and up: Teen PMH (once a year)
Asthma visit: Peak flow (per provider preference)

Well Child Check (WCC):
Vitals all ages
9 months – 5 years: HgB
12 months – 24 months: Lead level (up to 5 yrs if no lead result in chart)
2 years and up: BMI %
3 years and up: Vision and Audio Screening.
Peds DM*: 9 months, 12 months, 18 months, 24 months and 30 months (Anytime in between when peds DM is missed)
M-Chat: 18 months, 24 months and 30 months ( Anytime in between when peds DM* is missed)
16 yrs and up and all sport physical: collect urine for send out (perform urine dip on sport physical only)
